Overcoverage, Undercoverage...Why Can't I Just Get It Right!

I have been totally slacking on the blogging front and once again apologize. I just feel the need to be done with hip shit once and for all.
I am totally done with my original PT and now on to PT #2, who works closely with my OS.

I have been having some feelings of instability, especially when I bend down quickly. Also, I went sledding on Sunday and walking up the hill killed me the next day. I had a lot of groin pain and got really nervous.

I told PT #2 that I really want my full hip flexion, on top of the other things that are going on. He said that one of the issues is that my capsular shift is not doing what its supposed to do, and my hip feels "sloppy", when he externally rotates it there is almost no end feel, it just flops. He also felt that my femoral head was too anterior causing it to jam up into the joint, so he wants to try some things to reposition it. He ended up taping me into a little pelvic anterior rotation to try to get more coverage of the femoral head. After a few hours I had a lot of back pain so had to remove it. We are going to work on some exercises from now on to get that back into place.

He also worked on some muscles trying to get my hip flexion increased. There is a spot on the inside of my leg where the adductor meet the VMO that is incredibly tender, he worked it so hard but it increased my ROM...not without leaving behind some nasty marks though!

I go back to work on Monday...I am nervous about it...I will keep you posted!

I just found your blog today and can't thank you enough for allowing a window into what I am facing.
I have been diagnosed with FAI in both hips, started with my left almost 3 years ago. I tried numerous therapy options believing it was an iliopsoas issue. I just got the final diagnostic in June and while dragging the left around for so long in and out of pain I have torn the other. That diagnosis in December took me down a road of denial and being overwhelmed. Since I am pretty well informed as to the seriousness of this surgery especially its down time. Having had numerous other surgeries I am also more aware of what it really involves.
I am over believing I can survive in my very active and athletic life without it and have to get started.
My question for you is my dilemma in choosing a surgeon. I am new to this blogging so I don't know if it appropriate to talk about surgeons by name. I have traveled to Nashville which probably tells you who I am seeing currently but am questioning the need to look at some others specializing in Arthroscopies. I would appreciate your thoughts and again, thanks for taking the time to help others.

Hi. Can b quite confusing and overwhelming dealing with imbalances. I have to say wow on the uphill walking, again that was not in my rehab for about a year later. I would just get so inflamed. And even to this day if I have not been doing it for a while it will complain later. I have found out that the trick to keep the hips happy is to continually do a wide assortment of movements in your routine. Keep pushing girl.

Be confident in the Nashville surgeon you are seeking. He, along with Susie's surgeon, are among the best. Also check out www.understandingfai.com for a blog by someone who has a scope performed by Dr. Nashville. You'll be amazed at her recovery.

I was seen by the Nashville OS (self-referred) before being referred by an OS to Dr Kelly. I did not have a good experience w/the Nashville OS & left my eval thinking I would be stuck w/my hip pain. A few weeks later I saw Dr Kelly and had a completely different experience & more important, an answer & solution for my hip pain. In my case it seemed as though the Nashville OS was very conservative and didn't want to even try to address my problem because it wasn't a simple/easy case. That being said, I would rather be w/an OS who doesn't shy away from the more complicated cases because as you see from the hip blogs, sometimes what appears to be "simple" turns out to be a little more complicated in the end.

Hi! the OS in Nasheville is one of the best but out of the "top 3" he does have a reputation of being the most conservative, which is not neccesarily a bad thing. I see Bryan Kelly in NYC and personally feel that he is THE best. I don't know how much you have read but I am far from the norm when it comes to hip issues and he has dealt with all of my issues. If you tell us where you live it may be easier to recommend someone, but again, be confident in Nasheville if you feel he is right for you.

Wow, thanks for all the support, this is such a daunting time it is nice to think I won't necessarily being taking the trip aloneI
I am in Louisville Ky so Dr. Byrd is the most "convenient", I know his reputation and have met with him once and I would agree he is conservative and knowing how critical the surgeon and his whole team is to the process I am not certain he is my perfect match. I meet with him again in a week to get a better feel and try to determine which hip to attack first, since both are at the moment excruciating! I am open to traveling but concerned about the cost obviously. Dr. Kelly's web site and obvious fresh energy was top on my list just concerned about delaying any further and all the testing again. Anybody else in the area I might have missed? I know about the Atlanta team and Vail Co.

if you are close to st. louis you can try clohisy at wash u school of med. he too is quite conservative. takes a good 3 mos to get an appt with him, though unless you can get a referral from someone. not sure if st. louis is a closer option for you.

Anonymous, I would send your information to Dr. Sampson in San Francisco. He called me and had a phone consultation with me within days of receiving my information. I saw him and he did both my hips in the same surgery - saves on recovery time and money. I couldn't be happier with my results! I'm from CO and went to see him, shows a little insight to my feelings on the Vail team....

Thanks for the SanFrancisco Dr. I have not heard of him. WOW both hips at once?? I have not had that even suggested, it would sure help with my off work time to do that. If I may ask how you managed and how long were you without weight bearing? Also I am a 47 year old female in good shape [used to be excellent] I wondered if age was a factor in him doing both?

I walked out of the surgery center on crutches - so weight bearing right away. It was a slow walk and an interesting feeling but it worked! I thought it would be really difficult to walk after that surgery but it wasn't bad. I'm in my mid-twenties, so I'm not entirely sure if age is a factor. I don't think it is though because I know of two others in their 30s or 40s that have had the bilateral surgery with Dr. Sampson. I'm surprised you haven't heard about him yet - he's one of the top doctors. In my opinion he's the best (but shouldn't everyone's own surgeon be their 'best?!'). He doesn't take insurance so everything was out-of-network. He is VERY easy to work with when it comes to pricing. After giving me my 'quote' he asked if I needed him to lower the cost - I don't think many doctors do the same.
