The Ups and Downs and Then Some

Since becoming pregnant, I have stopped spinning, not bc I think in any way shape or form that it is harmful to the baby, but I have been to busy puking, lying in bed feeling nauseous, lying in bed feeling sorry for myself that I am so nauseous, or simply lying in bed bc I am too tired to move or do much of anything else. It is also unbelievably hot here, especially over the past few days, my feet have started to swell, which makes me REALLY unhappy.This has never happened to me so early in a pregnancy....but I digress.

My point is that my activity level has gone down...drastically, so my ROM is not as great as it once was and my hips feel tight on and off. This week I was at my breaking point with the tightness and succumbed to asking P for a quick PT session so I could walk like a normal person again. I have had my usual anterior tightness which limits my extension when I walk, so if I have to be anywhere quickly,I have to take smaller steps bc that bungy cord in my hip feeling is back. Not even 5 minutes of some anterior hip mobilizations and quad stretching brought me back to normal! YAY!

I'd like to resume spinning soon but I practically pass out every night once the kids are in bed, especially bc we spend all afternoon outdoors by the pool (I have not gone in for a while bc my bathing suits don't fit anymore...I ordered some this week, I hope they fit!!!)

I remember after one of my surgeries (I'd like to think it was #3, this would support my theory) I told P not to work on my scars, I wanted to see if there would be a difference long term in the appearance and quality of the scar. Given the fact that the revision scars are 4 and 8 months younger than the other scars, they look awful. I really do believe that these were the "experimental scope sites". They were worked on a little bit, P got mad at one point and said I was being ridiculous about it and he was not going to let the scars go to hell. I did let him work on them a little bit, but no where near as much as the other ones. These scars are not red and elevated, and one is pretty painful to the touch. Cosmetically, couldn't care less, trust me, no one ever sees them! And they never get sun, so that is not a problem. I wonder how that plays in with my "nerve pain"/ "invisible bruise" issue. I don't use any creams or scar therapy lotions, I think it is a waste of money, especially given how the other scars look and feel, but it makes me wonder, maybe PT really does work! hahahaha!! Just joking! It was a personal experiment, I always work on my patients scars but there is ever a way to really know of the results would be the same if I didn't, now I know and my work has been validated!!!

Quick Jeff update: He saw OS #2 who confirmed the labral tear diagnosis, despite the fact that he felt the quality of the MRI was pretty poor, and who added "we always order these with contrast". He did not see any FAI. I am concerned about this. Given my extensive personal research and "networking", I cannot think of a single person who has had surgery for JUST a labral tear who is doing exceptionally well. I wold like further investigation into the "no FAI" thing with possible a CT-scan, although my CT-scan missed my FAI.....but better to be safe than sorry.I still strongly believe he needs to make a trip to NY to have this checked out and addressed!

Revision Hip Arthroscopy/ Failed Hip Arthroscopy

I always wonder how many other people have fallen into this predicament as well. What I have noticed is that these are key words that often bring people to my blog. I have no way to really keep track, but often I will scan my sitemeter which will tell me the search words used by readers. It doesn't always work, for example, "Landsickness" through google brings up my blog, o the top 10 (I checked), but so do other key words, like FAI, hipscope, hip arthroscopy,psoas....and my all time favorite, "Bryan Kelly hipscope", my blog is #1 on google if you put in those words. This is quite embarrassing as well as pretty funny, if you ask me! (If he knew, he probably wouldn't agree!)

So, my point is, I was curious, who has had a failed hip arthroscopy? or failed FAI surgery? or failed surgery for a labral tear? Who has retorn their labrum?And even more importantly....why...what reason were you given??
Now would be a great time for all lurkers to share their story, so we can all learn from each other.
Me? An conservative surgeon who didn't want to take down too much bone if it was unnecessary. We found out the hard way that it indeed was necessary, but a happy ending for all involved parties!!!

Failed Hip Arthroscopy and Revision Hip Arthroscopy Articles

Purpose: The purpose of this study was to identify possible causes of failure of hip arthroscopy by reviewing the intraoperative and radiologic findings in a series of patients requiring revision hip arthroscopy. Methods: We retrospectively reviewed 24 revision hip arthroscopy cases performed in 23 patients (14 female and 9 male; mean age, 33.6 years; 1 bilateral). The review included indications for surgery, intraoperative findings, and arthroscopic interventions for both the primary and revision surgeries. Imaging studies, including radiography, magnetic resonance imaging, and 3-dimensionally reconstructed computed tomography scans, were analyzed for the presence of preoperative bony impingement lesions (e.g., femoral head-neck junction “cam” lesions or anterosuperior acetabular “pincer” lesions). Results: The mean interval between previous hip arthroscopy and recurrence of symptoms was 6.1 months. In 13 of 24 cases (54%), patients had no significant improvement at any point after the primary hip arthroscopy. The mean interval between the previous hip arthroscopy and revision surgery was 25.6 months. Unaddressed or undertreated bony impingement lesions were found in 19 of 24 cases (79%) and were identified on imaging studies before revision hip arthroscopy. A tight psoas tendon and corresponding labral impingement lesion were identified by arthroscopic visualization in 7 of 24 cases, for which partial psoas tendon release was performed. Eight cases of failed labral repair were addressed with labral debridement and removal of suture material. Of these 8 cases, 6 also had bony impingement, which was also addressed at the time of the revision surgery. Conclusions: Failure to address bony impingement lesions of the hip and a tight psoas tendon are key factors in unsuccessful hip arthroscopy and may require revision surgery. Furthermore, failure of labral repairs may be the result of unrecognized bony impingement at the time of initial surgery. Level of Evidence: Level IV, prognostic case series.

Background: Hip arthroscopy has become increasingly popular; however, little is known about revision hip arthroscopy.
Hypothesis: Revision hip arthroscopy is associated with unaddressed femoroacetabular impingement. The purpose of this study was to describe reasons for revision hip arthroscopy.
Study Design: Case series; Level of evidence, 4.
Methods: Between March 2005 and March 2006, 37 revision hip arthroscopies were performed by the senior author. Data were collected through retrospective review of clinical and operative notes.
Results: All patients required revision surgery because of persistent hip pain. There were 25 women and 12 men with an average age of 33 years (range, 16–53 years). The average time from prior surgery to revision was 20.5 months (range, 2.9–84 months). Common findings among patients needing revision were hip pain, decreased range of motion, and functional disability. The average modified Harris Hip Score was 53 (range, 22–99). Thirty-six patients had radiographic evidence of femoroacetabular impingement at the time of revision. Revision procedures included 34 (95%) for femoroacetabular impingement, 32 (87%) for labral lesions, 26 (70%) for a chondral defect, 23 (62%) for lysis of adhesions, and 13 (35%) for previously unaddressed instability. Two patients had total hip arthroplasty after revision, and 3 patients required further revision. Of the remaining 32 patients, early follow-up was obtained on 27 (84%) at an average of 12.7 months postoperatively (range, 6–19 months). Outcomes showed patients regained some of their lost function within the first year.
Conclusion: Patients commonly required revision hip arthroscopy because of persistent impingement.

One Year Anniversary

Today marks exactly 1 year from my first ever hip arthroscopy. This was supposed to be the ONLY surgery but as luck would have it, it opened the flood gates and resulted in 2 more surgeries, bringing me to a whopping 3 hip arthroscopies in only 8 months. Its like winning the lottery! hahaha!

1 year ago today, at this time, I was most likely downing a Vicodin and in a ton of pain. I actually remember very well that I decided (stupidly) to sleep through the night and went 8 hours with NO pain meds. When I woke up in the morning, I lunged for the bottle and took 2. That began the vomiting with hipscope #1. I learned after that and never went more than 4-6 hours without drugs. I even went to to PT the next morning (absolutely barbaric) and lunch, but that was where the fun ended. It was back to praying to the porcelain god for me!!

I ended up getting Compazine (pretty light weight for me) and Ultram because my pain was so bad, I stayed on the Ultram for 2 weeks. I was in a lot of pain after the first scope. I imagine that it was due to the f---ing impingement remaining in my hip. I noticed a huge difference immediately following scope #2, even from day 1. Its a shame that my left hip didn't become painful first.It would have saved me a lot of time, pain and a hell of a lot of aggravation.

One year later, the results speak for themselves. Yes, there were a few extra (not so) minor bumps in the road, but the important thing is the end result, and my hips rock (for the most part)!

Too bad I didn't begin my blog 1 year ago, but like I have said, I didn't think there would be anything to blog about!

Ride On!

The weather was beautiful today so I piled the kids into the double stroller and went to the park. For those of you who do not remember, pushing the double stroller was what eventually landed me in Dr Kelly's OR 3 times.

This is one of the first times since my revision scope that I remember pushing the double stroller all the way to the park, so it felt like an accomplishment, plus, I like having the experience and knowing that I can do it. My right hip felt weak while pushing. Then again, I am pushing 35lbs of stroller and about 70lbs of kids. I also had discomfort in my trouble spot. The area along the front of my thigh along all of the scope sites, where I have supposed nerve damage. It didn't hurt but I could feel that it was an area that was not done healing.

As for my arm, the tingling has diminished.I have decided to see my acupuncturist this week, I will let you know what happens.The good news is that I don't think it is too serious and the tingling has centralized to my elbow. The bad thing is that my elbow hurts now. I think it may be the way I am doing a manipulation at work so I will have to be careful.

In an unrelated story, have you been following Carlos Delgado's story, from the NY Mets? He was flown from Florida to NY to have an MRI at HSS, the findings were that it is not serious, it is just an impingement in his hip.

WTF???? Not serious? How many of you have been sidelined bc of a simple "hip impingement"?

3 Months Post-op Hip Arthroscopy Revision

I was on the phone with hipster Amy tonight. We have a 'date' tomorrow at Dr. Kelly's office, for her 2 week follow-up and my 3 month follow-up. The topic of conversation turned to successful surgeries! Finally, someone who can understand my joy and elation, and smiles when no one else knows why. Joining us on our date will be 'L', who loves bones. I promised her she could see my x-rays, she is ecstatic.

Since my life has returned to pre-hip issue status for the most part, my old knee issues have returned. Way back in college and PT school, I had such bad knee pain, I had to ice after each ride. I am approaching this again. I really can't handle another orthopaedic issue right now, or ever! The good thing is the knee pain doesn't make me crazy, or do crazy things! I need to ice and get a grip on this, before it gets out of control.

J has been away on business, he was in Mexico last week. He decided to come home early tonight, conveniently missing bath and bed time for the kids, but early enough to want to know what I had made for dinner. His bright idea of the night came as he was watching a Tivo'd episode of The Biggest Loser. "Why don't you gain a ton of weight so that we can go on the 'couples' version together". For those of you not familiar with The Biggest Loser, it is a reality TV show featuring heavy contestants who battle to lose the most weight in the season. He is obsessed with it, and wants to go on it, but not alone. So, his idea is for me to join him! Fat chance!

12 Weeks post Revision

As I was walking home from taking L to school this morning, a great thought crossed my mind. I have no pain, no discomfort, and no tightness. I don't feel a thing in my hips (both sides), it feels as if I did not have surgery! Whoohoo!!! I have been waiting for this day for over a year, to be completely pain and discomfort free! I know that this is probably not permanent, and there will still be periods of discomfort down the road, but what a great milestone to hit! I am back on the spinning DL for the moment, this may be why I am feeling so great. Battling yet another sinus infection, which leaves me feeling completely drained and exhausted. But I can't just stay off the bike forever, eventually, it will stop making things tighten up, and I do think that each ride is better than the previous one!

As Tight as a.....

I can't think of a simile, but my hip is TIGHT. As I was riding tonight, I got into some standing sprints for the first time, but it felt as if there was a cord wrapping tighter and tighter around around my hip with each revolution, until I couldn't take it anymore. I kept saying to myself, 'it is not your problem, P will deal with it tomorrow...' but hell no, it is my problem now! Especially since Jk is asleep and I have no one to sit on my leg and stretch me!

Interestingly, I had a pretty shitty day on Sunday and Monday. Ironically, those 2 days were really really cold, the kind of days where you don't want to go outside unless absolutely necessary, the days where you drive down a street in Manhattan and there are no people on the sidewalk. Why is the cold bothering me so much???? I was between Tylenol and Darvocet, I was going to start with Tylenol and then take a Darvocet if it didn't help but it must have bc I never went for the stronger stuff!

I am going ice skating again tomorrow with hipster Amy, who has been banned from the ice by Dr. Kelly, but L really wants to learn how to skate! I am more nervous about freezing my ass off than breaking something since we are going to the Wollman Rink in Central Park, the indoor rinks are closed tomorrow or don't have public hours, I already started packing a bag with extra socks and sweaters! You can take the girl out of Miami, but you can't take Miami out the girl (did I ever mention I was from Miami, and have been 'stuck' in NY since college???)

On a positive note, I think I had the best ride tonight since my revision scope! YAY!!!
Just curious....should I still call scope#1 a 'failed hip arthroscopy' or 'part 1 of the job for my right hip'!
Also, I think I have to change my profile since I mention the 'twisting/popping' incident and really, it has nothing to do with anything, other than sending everyone down the wrong path on the ever so fun investigation of 'why does my hip hurt AFTER my arthroscopy?"
(I have to stop exercising at night, I have way too much energy)

Just To Keep everyone on the Edge of Their Seats...

My hip hurts. I am pretty sure the muscles are acting up, but I am f___ing annoyed. See, we are past this. It is like a relationship, we should be done with the awkward, getting to know you part, and be on to the 'romantic walks on the beach', or rock climbing and spinning together. I have yet to 'poke around' the area to figure it out. Yes, instead of sitting my lazy butt in front of the computer, I should be stretching, but I had such a great ride tonight I want to relax a little.

Maybe it is wishful thinking (I am rambling again...I know), but before the FAI was bulldozed out, I could literally take my index finger and point to where the pain was. This pain is readily identified with the entire pinky side of my hand, from the tip of my finger to my wrist, right in the joint crease.

Also, pesky adductor is acting up again. I need to get off of the computer and stretch....I did make an appointment to see P on Friday morning, which means I am, once again, not a PT dropout. Lets keep me at PT PRN status. I actually received a prescription from a doctor today which said PT at 'my discretion'! So I am doing the same....but for myself.

I play this game called 'what if I was my own patient', to see how I would handle this muscle issue. Firstly, I would not approve of rock climbing or ice skating yet. Maybe spinning, but low-level. Definitely not what I have been doing some nights, like tonight. Did I mention I hurt more now that I am off of the bike than I did before I got on? Oops, I left out some important details. I also had a crazy day at work plus I cooked dinner for 2 other families tonight. Ok, so maybe I don't practice what I would otherwise preach. But this hip shit is getting really old.

Landsickness

No, I am not being facetious. I have friggin' land sickness. I have been off of the boat since Monday morning yet my body feels like I am rocking. Clearly, it is all in head, or brain, or ears...or something! I have discovered these acupressure sea bands that seem to be helping, but I have to always wear long sleeves to cover them up for fear of looking like a bigger lunatic than I already am!

The cruise was great, rock climbing was incredible. It was definitely a personal achievement, not bc I need to climb walls to feel accomplished, but my hip can handle it, which makes me feel awesome! Kudos to P and Dr. Kelly for making it happen!

At this point, things are great. I can feel like there is 'something' in my hips, the left one (6 months out) feels more 'confident' when I walk, run, ride....The right still has occasional adductor issues. Time will heal!

I am in limbo with PT! The day I left for the cruise was the same day as I had the horrible capsule issues after scope #2 (7 weeks, 5 days). That day, the joint capsule started tightening. Shit, I thought, its happening again. But I tried to stretch it as often and discreetly as possible (in the airport and on the plane) and warded off an attack by my angry joint capsule. I did take Darvocet that day, just to be sure I wouldn't have issues. I didn't take any again bc I was on vacation, without the kids, so drinking was a must :-) I even went off of my antibiotics for the weekend (I know, me being smart again).

I saw P on Wed., we focused mostly on joint mobs for the capsule, we are definitely being preventative on this. I have no appointments set up for the future right now. We will play it by ear. I have not been on my bike since returning due to my dizziness issue, which sucks, although I think it is better than nausea bc I can function for the most part, and I am not throwing up (knock on wood). If by Sat. I still feel dizzy, I will ride anyway. My goal for 2008 is not to see doctors...dizziness included!

When I first began my 'hipscope research', there were so few success stories. I am proud to be a success story x 2!

6 Week Update

I can't believe it has been 6 weeks since my revision surgery. I see that there are a lot of new readers, so welcome, I hope you are enjoying this pathetic rendition of what my life has become!!! J/K!! I am actually really happy!

Just to recap- I had a revision hip arthroscopy on November 12, 2007 after my original scope on 3/5/07 failed secondary to residual FAI (femoroacetabular impingement). I originally had a labral tear and cam and pincer FAI, the initial surgery did not address the cam FAI (long story). In the long run, I am very happy my surgeon was not overly aggressive because you can always go back in and take down more bone, but you can never put it back!

So, 3 hip arthroscopies later, I can say that it was all worth it because I am in a much better place.

Just yesterday, we went to the NY Botanical Gardens holiday train show, it was our 3rd year going, I guess it is a tradition now! I have vivid memories of going last year. My hip hurt so much, and this was before I had been 'officially' diagnosed. I was so anxious at the time, as well as frustrated that the simple task of pushing my stroller hurt. Yesterday was a pleasure, for my hip anyway. We waited in line for over 40 minutes just to get in, and then the entire show was done in 'line form'. The kids loved it, and we had a blast. I never once thought about my hip!

I cannot remember if I have joint pain. Clearly, it is not an issue. What is still an issue is the 'invisible bruise' on my thigh.It still bothers me at work, when leaning with that leg against a treatment table. I have learned to fold a towel in between me and the table to help add more cushioning and protection there. The scope sites have been neglected this time so are tender to the touch, they are in the vicinity of the 'invisible bruise', so the entire area is tender to the touch, still.

My numbness is still there, and most recently has began to burn. It is not constant, and has only happened a few times, but is quite irritating. The alternative would be to try a heavy duty med called Lyrica, but the side effects are pretty intense, so I can only imagine what that would do to me, so no thank you! Hopefully it will not get worse than this.

My pain in the ass is better, but my piriformis is still in spasm, not as bad as last week though, phew. I had PT today and had P work on the scars and 'invisible bruise' and stretch my psoas, which is tight (but not so tight it needs to be released!!!)

I am supposed to go back to Spinnning today/ this week as per my own goal but I have a pretty bad sinus headache/ infection so it will have to wait a few days until it clears! Tomorrow is a new day though!!!!!

My left side is almost 6 months out, doing great, with just some pretty noisy popping/ snapping. The noisiness is painfree so I am not in the least bit concerned.

On a Nerdiness Scale of 1-10, I have reached an 11

Today, as both kids napped, I picked up a PT journal (this is not where the nerdiness ended). I began reading an article:

It was about rehabbing a patient with a revision total hip replacement. As I reached the end, the authors quoted another article:

The first article quoted the second article saying that the authors found that the primary reason for revision hip ARTHROPLASTY (i.e total hip replacement) is impingement. As I was reading, I said, out loud, WTF??? That is not what that article says, I know that article like the back of my hand and they don't speak anywhere about arthroplasty, only arthroscopy. Yes, we all know that the primary reason for a revision ARTHROSCOPY is impingement. The authors misquoted Philippon et. al. My husband thinks i have gone off the deep end...again, he can't believe I caught that, and that I plan on emailing the authors regarding their mistake!

My 1 Year Anniversary

Today marks the 1 year anniversary of when my hip saga began. For anyone just beginning their journey, I can assure you that in the hands of the right surgeon, there is a light at the end of the tunnel. I am going to do a flashback to November 28, 2006.

Shit, my hip hurts. It hurts when I drive, and especially when I push the kids in the double stroller. It is a really annoying pain. It feels like something is in my joint. My friend/ PT is coming into work around 3, I will ask her to work on it and hopefully get some relief. She showed up and I told her I was dying of pain, she did a painful release on my iliacus/psoas, I felt better immediately, but in the car on my way home, things got bad again.

I went into work the next day, again, in agony. First I had one therapist stretch my hip flexors and ITB, thinking this may help. Then I had another one check it out. She said there is definitely some kind of impingement. Then I asked P for help. He was really busy so I waited until he was done seeing patients. What makes the day so memorable is that it was Jk's 1st birthday, and I was having a small dinner party, and lacking a few key ingredients. I used P's full schedule to run to the supermarket near my office but didn't find what I was looking for (which I now don't even remember what it was). P did all kinds of releases, ART, mulligan mobs...It felt good...until I was in my car again. Shit. What is this????

I got home, L had ballet and there was no food for my party! I ended up ordering pizza, Jk didn't know the difference! After we had cleaned up and the kids were in bed, I hit the computer. I had a feeling I had a labral tear, and the word impingement reverberated through my head. I think I first looked for a surgeon, before I did anything else, that is me, always thinking ahead! I was trying to remember when I had seen a hipscope, and who did them, and no one was popping into my head. At the time, there was a hipscope patient in my office and she was a wealth of information, but I wasn't willing to see her surgeon (he was in NJ), I knew it had to be someone at HSS. So I looked and searched, and researched labral tears, FAI.....Not just that night, but every night after that for weeks. After a week or 2, I decided this wasn't getting and made an appointment with Dr.Kelly. I was between him and another surgeon. I had emailed a professor of mine about this and he had recommended against the other surgeon for personal reasons. So Dr.Kelly it was. If things didn't go well, then I would pursue other options, it was just a consult, right. Hopefully he would order an MRI and get to the bottom of this. The rest is history....but in a nutshell...

PT, Finally!

I was late for my appointment, but I did eventually get there!! It felt like I hadn't been there in...forever! P asked what we were up to, I laughed bc we weren't up to anything really! So I rode the bike for a while, did some side to side on the wobble board since according to P I have a 'mild to moderate Trendelemburg', so when I walk, my right hip drops down bc it is not strong enough to support me yet.
I did bridges, hip adduction with a ball, hip abduction with a green theraband, lateral leg lifts. Then P showed me a progression for a new exercise he had learned at a course a few weeks ago (we are always at continuing education courses, my boss says he likes to hire PT nerds, and we so are!!!), 5 lateral (sidelying) leg lifts, then hold the leg up, 5 knee bends, then 5 times rotating into IR/ER, all while holding the leg up. I was a little shaky but I did it. Then some seated hip IR/ER with a ball in between my knees and a band around my ankle. Finally, isometric hip abduction against the wall, balancing on the right.

It sounds like a lot but it really wasn't.I am not sore at all. He did a Mulligan mob for hip extension, I put my left leg up on a chair and lunged forward, so the right went into extension, he put the Mulligan belt around my hip and applied a lateral distraction. He also did a lot of quad and hip flexor stuff and some prone anterior mobs, also for extension. I am ready for a nap!!!

Details, Details,Details Part 2

As promised, here is the rest of the 'story'.

So the last thing I remember is the anesthesiologist asking me to sit up and then passing out. I am really scared of spinal needles, I have horrific memories of the pain of the spinal before my c-section and the epidural during childbirth with Jk, I like that I was out fr it during my first 2 scopes and wanted the same this time.

I was awakened in the OR, I think, and I asked what time it was, someone said 10:30, and then I fell back asleep. I remember waking up in the PACU again, maybe Dr.Kelly had come to speak to me as I really made it clear that I wanted to speak with him before going home. He had told me to make sure not to leave, plant my feet and don't move, until he came out of surgery!!! He tried to show me the scope pics and some x-rays, but seriously, he could have been telling me about the football game from the day before, he did say something about scar tissue and the cam bump, then I fell asleep. My spinal began wearing off and my pain was bad. I was given some Dilaudid, I slept some more. Then the PA from pre-op came to check on me, I remember saying 'I need pain meds', which got everyone jumping. The nurse said she had given me all the Dilaudid ordered, so did I want to try and eat something and take Percocet since she would have to ask the anesthesiologist if I could have more Dilaudid, and he was in surgery. I'm sure by now you can guess what my response was: "Page Him." So she did, and I got my drugs! I slept more, and more. I think I ate some crackers at some point and drank some gingerale to take the Percocet, then I slept more, I was really really lightheaded and dizzy. I don't remember if I got more Dilaudid after that. Dr. Kelly came back again, he asked if I remembered speaking to him earlier, I said yes, but I didn't remember what he said. HE told me again...took down the cam, scar tissue, and the psoas was fine 'you were right, I'm glad I listened to you.' He also said had he done my left hip first, he would have done the same on the right, cam and pincer in one shot. I asked, 'so I am not crazy', to which he replied 'that I can't say!' While he was there, he checked my dressing and decided to change it bc my incisions had ozzed I guess, I was in no condition to really care though. The nurse also requested he order more fluids since I had all that Dilaudid.Then I fell asleep again.I just couldn't stay awake but I really wanted to get out of there and go home. I finally called over my nurse, who btw, was incredible, she took great care of me! I told her to get me up, I want to go home! she asked if I was sure, she was nervous bc I was so lightheaded. I said we'll take it slow. So first she sat up the bed, then I sat on the edge for a while, I was close to the fine line between passing out and not, but I made it. She had to take out the epidural catheter from my back, then I transferred to a reclining chair on wheels. I was taken to the bathroom, you cannot be discharged without demonstrating that you can urinate. in the past, by the time I get out of bed, I am ready to explode, this time I wasn't, but I knew I had had a lot f fluid. I got to the bathroom and had no urge to go, but sort of forced myself to. I had a FULL bladder, so it was weird to not have any sensation. I mentioned it to the nurse but she thought nothing of it. The same happened when Jk was born...it resolved. I was then taken to Phase 2 of recovery where you get dressed and have PT. They left in my IV because I didn't look too hot! I now know the PT's and they were really nice to me. I got dressed and took a short trip around with my crutches and was allowed to be discharged! It was around 4:00 I think, surgery was at 8AM! There was another one of Dr. Kelly's patients in phase 2, he obviously had his surgery after mine since I was first, and we were being discharged at the same time! I would make a terrible drug addict, I would be too tired and sick to enjoy the high!! I guess that's a good thing. J picked me up in the car and we went to CVS for my drugs.

I made it into CVS but began feeling lightheaded, so I sat down. CVS was out of Percocet, I kid you not, and hadn't filled my Zofran bc it was for 20 pills but my insurance only covers 12. We got the 12 and went home sans Percocet, but I had some leftover from last time. Good thing I keep a nice stash!! I inhaled a Zofran in the car and made it home with no incident!

I spent most of the rest of the day in bed. I went to the bathroom shortly after arriving home, I looked at my leg, shit, the bandage had soaked all the way through with blood. This had never happened, I guess that's why he changed it earlier. I called and spoke with Dr. Kelly, he said to have J go buy gauze and tape, change the dressing and apply pressure. We changed it and I fell asleep. Arianna called around 9 to check on me, I think it had stopped bleeding, she said numb up my hip and sleep on that side to apply more pressure. It worked, it hasn't bled since.

I woke up around 10:45 needing a Percocet, J brought me some soup and I had the pill, was up for a bit and fell back asleep. I guess I am making up for all the nights the last few weeks. I no longer have anxiety, obviously it was over this, my blood pressure yesterday morning was 140/82, way too high for me!

I think we are in the clear, other than some serious butt pain from the traction, I have only surgical pain, no 'groin pain'. I will definitely keep you posted in my usual fashion!!!

Post-Op Day 1

I know I promised to recap surgery day but I will post it later, it is too much info for me to write about in my Percocet haze.

I slept well last night, woke up and was able to have breakfast, no nausea. I then removed my dressing, the scope sites were not oozing and there was only a little bit of blood on it. I applied my waterproof bandages and showered, then removed them and applied regular (elmo) bandaids (that is all we stock here!!!). I was able to get dressed independently except for my socks which my mom helped me put on. Then we were off to PT. I think I took a zofran before leaving.

In the car, I took a Percocet. PT was basic. We took some ROM measurements. My flexion is same as it was pre-op, 112'. IR 30', it was 36' pre-op. This is great. What is limiting me is the HUGE amount of swelling. This is the worst it has ever been. MY leg has tons of bruising, I think Dr. Kelly must have been like 'I'l show her', hahah! J/k. As P was flexing my hip, I felt like my thigh was going to rip open at the seems from the swelling. My IT band feels tight as do my external rotators, thanks traction!!

My pain is not terrible, but I am taking percocet religiously. If I lay in bed and don't move, I am good. Moving hurts a little but I have so much more active motion than I have ever had this early post-op. I can do a heelslide without a problem and getting in and out of bed is not as hard as it has been. Again, 'wow, PT really works'!!!

After PT, my mom and I went out to lunch. As we sat down and the food came, I got very lightheaded so quickly ate half a sandwich, it helped. The plan was to go to CVS after lunch to pick up more bandaids, colace and percocet (can you believe the pharmacy was out of percocet last night....good thing I had some left over from last time) but I got soooo tired I had her take me home and I napped for 3 hours.

Now I am in bed, just chilling! I have phone calls to return and I should ice, I forgot to put my stolen icepack in the freezer (I took one from work today).

Details, Details,Details

I am in bed, took a Percocet about an hour ago. No one else is up in my house yet so there is no point in geting out of bed.
I had to be at HSS at 5:45AM. I had already filled out the majority of the paperwork last week, so I waited. In the meantime, I met the people from the magazine who were doing the photoshoot. They didn't call me in until after 6:30. I had to take a pregnancy test (negative, phew), and put on one of their cool hospital gowns with the heater attachment, and the 'hip scope breakaway' shorts! The camera crew came in to take some pre-op pictures. The PA came to do his mini pre-op physical around 7, 7:15. J was still in the waiting room. I was getting nervous bc in the past, everything has been done by 7, but hey, it wasn't like they were going to start surgery without me, right!
Next came the woman who washes the surgical body part, so she washed my hip. Then J was allowed to come in and we hung out for a bit.

Anesthesia came next. It was not Dr. Jules, the one I had requested. But I remember Dr. Kelly reminding me that all the anesthesiologists are great, so I was ok.He asked if I had had issues with anesthesia in the past. I explained that when I had a spinal with scope #1, it took me a long time to recover, so scope #2 was done with a combined spinal epidural, much better. So he said he would plan on that.
The nurse was next, almost 7:45, we were running out of time. She had to go over my medical history again, then start the IV. She wanted my to lay down, I assured her I wouldn't pass out, but she insisted.

While she was doing her thing, Dr. Kelly showed up to 'sign' my hip. I asked him to please write 'do not release psoas', in case he forgot! He said he would remember, and wanted to know if I was going to have the psoas police in the OR!! He even showed me the consent form, he said 'look, it is not even on here', so I said 'good, bc I don't consent'. I asked if I am his biggest pain in the ass patient, he said not even close, I just know too much. I didn't have a response bc just at that second, the nurse inserted the IV needle which silenced me! He said he was just going to go in and remove scar tissue, adesions and obviously the cam bump. Ok, fine,I could deal with that.

The OR nurse came next.She looked at my hip brace and asked where the rest of it was? Huh, thats all I have. What about the posts and pillow?Huh?? That's all they gave me. So off she went, super annoyed, to find whatever it was that was missing (I still am not sure). she came back to bring me to the OR, 'we are 5 minutes behind' she said. I left J, and we went to the OR. We passed someone who was looking for whatever it is she was missing.He said 'I told Dr. Kelly', to which she got clearly annoyed and said 'don't tell Dr. Kelly'. I assume they found it bc we proceeded into the OR. The camera crew was in there with someone showing them all the scope tools, I was trying to get a glimpse, but they needed to get the anesthesia going. I was laying on the table and the anesthesiologist gave me something to relax me, and asked if I could sit up,I told him I didn't want to feel the needle go in, he told me he would make my skin numb, and then I was out!

Some More Tidbits

I am copying the email I sent out earlier to the 'groups' and adding some more details.

Hi everyone!I am home and resting after my revision hip arthroscopy ( bringing me to a total of 3 scopes). My worst fear was that there was really nothing wrong, and I was 'making this up'. My second worse fear was that my OS would be right and that this was all psoas. We had a lot of discussions/ arguments over this and finally settled on him promising not to do any type of release on my psoas.Well, to my surprise (not really), my psoas was fine, it is nice to actually hear a surgeon say 'you were right, I'm glad I listened to you'. He did shave down any residual cam impingement (pincer had been taken care of in scope #1). Being that I am argumentative and stubborn, I have gotten away with coming home with no cpm or gameready this time, my kids use the CPM as a ride-on toy and the gameready is just annoying for anyone taking care of me to use. I promised to be compliant and ride my bike and ice with regular ice.So...I had a large cam decompression, scar tissue removal, synovectomy (he said the joint was really inflamed). I think the labrum was fine, there were some adhesions I think....we spoke twice but I was really drugged, I will get the op report as soon as I can. He also said something about the cartilage being soft. I woke up and once my spinal had begun to wear off I was in awful pain, worse than any other surgery. I maxed out on the prescribed amount of dilaudid so the nurse had to page the anesthesiologist to see if I could have more. I was also given zofran and pepcid for nausea in the OR, and zofran to take home.I was still REALLY nauseous earlier. I also had a lot of drainage from my incisions, had to have the dressing changed once in the PACU and when I got home it had soaked through again and changed it again.As I am writing, my husband just said that he saw the before and after x-rays of the femur, before looked like 'rocky mountains' and after was nice and smooth.The pain is there but the hip is moving well, I think I attribute this to some serious pre-op PT. I went in really strong.Before I continue rambling in my percocet/ dilaudid induced blur, I will stop!Thanks to everyone who has been there to listen to me vent and complain and tell me I'm not crazy!!!

Thats the email. It is 12:30AM. I slept a few hours earlier, then I woke up and needed percocet, so J brought me some chicken soup to eat first. Percocet and empty stomach don't go well. I am also popping Colace like candy. The Percocet makes me extremely dizzy and lightheaded, but it is the better alternative to Vicodin, which makes me throw up like a madwoman. I haven't taken another Zofran yet, I probably will in the morning. I am feeling itchy all over, I will do a quick search on the meds they gave me and see what the deal is. My thigh is numb in the quad area. I will see what else is numb in the AM when I remove the dressing to shower (shhh). There are 3 incisions, I cant tell if they are over the old scope sites, I asked Dr. Kelly but don't remember what he said. I hope I can sleep tonight.
I am also having bladder issues. I am not having the urge to pee, even when I know I have to bc I drank a ton.I sort of knew right away bc in the pas I ahve woken up from surgery and really have to pee, this time no urge and they ad t hang an extra IV bag to be of the extra Dilaudid. I finally went to the bathroom and sort f had to force myself to pee, alot came out but with not a lot of force. Something similar happened when Jk was born, it resolved on its own in time, and I am sure this will too, but it is just one more annoyance for me at this point.
I think that is all I can think of for now!

Timeline

I know that my blog, up until now, has been out of order and maybe confusing at times, so I would like to make a timeline of events to make it clear what led up to 3 surgeries.
November 2005- Jk was born (2nd child, boy) via VBAC, I felt hip pain, but what didn't hurt! Pain went away on its own.
November 2006- Hip pain came back, ignored it for 2 weeks, on Jk's birthday (how ironic) pain was AWFUL, had 3 different PT's work on it, PT only made it worse
Dec 2006- Exercise makes my hip hurt more, manual therapy makes my hip hurt more, time to look into seeing a hip specialist
Dec 2006- Don't get to bed until after midnight, for weeks as I am doing tons of hip research and investigating specialists, come across 2 articles in JOSPT that are encouraging. Found hip labral tear and FAI web forums, learn a ton from other people's experiences
Dec 2006- Choose hip doc, get an appt one month later (see earlier post)
Jan 2007- See hip doc, agrees with self diagnosis, order's MRI (no injection, he believes his MRI machine is good enough to pick up a labral tear without contrast)
Jan 2007- Receive call form hip doc's PA, MRI showed tear and FAI, go get diagnostic injection and make an appt to see hip doc 2 weeks after
Jan 2007- Have injection (see earlier post), injection provides immediate relief
Jan 2007- Go to Disney World!!!!
Feb 2007- See hip doc again, he is pleased that I got relief with injection, lets hold off on surgery, maybe the injection was enough. I don't agree, I want to get this over and done with, as long as he feels comfortable operating. He says "oh, I feel comfortable, I just didn't know if you did!!"
Feb 2007- Schedule first hip scope for March 5
March 2007- First post-op visit, 1 1/2 weeks later, things seem to be going well, I mention that my other hip hurts, he says its compensation, we leave it at that, he wants to see me in 6 weeks
April 2007- 2nd post-op visit, at this point, the other hip is becoming a major issue, I make it very very clear to him, most importantly, it is affecting my rehab on the operated hip, so I want it injected, just for pain relief, he agrees and wants an MRI too
May 2007- 7am MRI followed by 12pm injection, relief with second injection
May 2007- doc calls, MRI positive, looks identical to MRI on other hip. He generally will operate a second time 8-12 weeks out from the first
May 2007- after 3 sleepless nights, I schedule the second surgery, they will be 17 weeks apart
June 28 2007- Twisting motion while applying make-up causes loud, painful pop in right hip
July 2 2007- Left hip arthroscopy, findings: torn labrum, cam and pincer impingement,loose bodies
July 18 2007- 1st post-op appointment, right hip pain is compenastion, left is doing fine
July 26 2007- Right hip is unbearable, call doc, decide to schedule MRI
July 31 2007- Right hip MRI- shows farying of labrum anteriorly and Cam impingement, doc says it is nothing to worry about, do I want injection? I will think about it
August 28 2007- Finally have arthrogram (intra-articualr injection), got 3 painfree hours
August 29 2007- See doc, discuss cam impingement in right hip, he tells me that sometimes he needs to do revision surgeries in cases like this
September 2007- Discuss situation with doc, he believes it may be my psoas as well, schedule CT scan, scan inconclusive
October 2007- Doc strongly suspects psoas involvement, I still disagree, we agree to try a psoas injection, bad reaction, mild pain relief
October 2007- See doc post-psoas injection, beg him to put me out of my misery, we agree to set a tentative date for surgery and wait 1 week to see how I fare with injection
October 2007- Injection did not help, Nov 12 2007 date set for Right hip revision arthroscopy
November 12 2007-Right Revision Hip Arthroscopy, cam impingement, scar tissue, synovectomy
April 2008- Get pregnant with Zk
January 2009- Zk is born, once again via VBAC (drugless too)
June 2009- After sitting in Indian Style for 30 minutes, I procees to stand up and hip is too painful to stand on
June 2009- MRI of R hip shows increased signal in capsule
July 2009- Intra-articular injection of right hip gives approximately 2 days of relief
September 2009- PRP injection of the joint capsule gives no relief
September 2009- BK tells me he thinks I need exploratory surgery (this will be surgery #4)
September 2009- I freak out, BK agrees to try one more injection
October 2009- Synvisc + Cortisone injection scheduled for right hip

November 2009-Schedule Hip Arthroscopy #4 (#3 for right side)
November 2009- Hip Arthroscopy #4 for possible HO, labral tear and capsular shift
February 2010 Hip begins to feel unstable, joint pain becomes awful at times
February 2010 I begin to become concerned that I have a TON of hip ER, despite capsular shift
March 2010 MRI to see what the issue is (this time...sigh)
March 2010 BK thinks it may be my psoas or capsule or scar tissue
March 2010 Discuss possibility of dynamic ultrasound and/or dynamic CT scan
