Funky Popping Going On
Yesterday took the cake! Try to picture this. I was lying in bed, on my right side, very close to the edge of the right side of the bed. I let my right leg slowly hang of the bed (don't know what I was doing) and pop, pop. 2 pops in the hip (psoas). So I tried to re-evaluate what had happened, and came to he conclusion that I had internally rotated the right leg and then it popped. Which led me to wonder if it really was the psoas, or rather some old scar tissue.
I know, I have way too much on my hands!

Woe Is Us Part Deux
If I ever doubted that the entire body is connected and works as one impressive chain, today would have changed my mind.
I have been having incredible muscle pain over the past few days, it hasn't been this bad in a while. Yesterday and today I have been dealing with the right side (2 scopes), I feel my psoas pulling at my vertebrae again, and then I get awful pain on medial side of my thigh, not quite groin. This is palpable too. I am so stiff and clicky as well.
Yesterday was a sore day. As I was waiting for the elevator to go down and pick L up at school, I started stretching and trying to get my hip to extend, and hopefully stretch, or possibly break up scar tissue. I got it to pop, but then it hurt. It did get better at some point, I don't remember exactly when or what happened. Which leads me to beleive it was scar tissue
Today, that side was a mess again. I felt like my hip was so tight I was losing extension again, like in the early pre-op days. I did the "stool stretch", where I put my knee on a rolling stool and stretch my leg out behind me. It felt good, I got up to walk, and my right knee was a disaster (i.e. the good knee), I was limping for a while. This too, cleared up, but the hip remains tight.
Like the uncompliant,impatient patient that I am, I had a good ride on my spin bike tonight. I know this is not helping the psoas issue since I am sitting, and spinning my legs, fast, in a small arc of motion. The psoas just winds tighter and tighter.
I have been strtetching it, still, and still doing my abductor and adductor exercises, as if this balances out the "don't spin" part.
Time to get on the floor and stretch stretch stretch!!!

You Know its Not Going To Be A Good Day When......
My psoas has been giving me issues lately, on both sides, I think. It is hard to remember sometimes, although today we are talking of the left side. Operated only once in July 2007. Last night I began to diligently exercise adductors and abductors on both sides to help my "ailing" knee and "psychotic psoas". I also have been stretching the hip flexors a lot more lately. But this morning, I felt like I made a 9 month backtrack in muscular issues (Joint is still good, you can all breathe a sigh of relief, joint is actually amazing and I am super thrilled, which is why I save my muscular issue complaints for when they are really bad). My iliacus is a mess for some reason. Iliacus is the top portion of the combined muscle called "iliopsoas", iliacus+psoas=iliopsoas. It is found posterior to the ASIS and can be palpated by bringing your fingers just under the ASIS. I have not attempted to manually work on it, but have been stretching it as much as possible (all hip flexors). I stretched before I took L to ballet, after I picked her up from ballet, while she was at a ballet birthday party, and before we left to go to her birthday party! I still have a lot of pain localized to that area. It is an improvement from this morning, I think I could almost feel it pulling on my vertebrae, and that is why my back was bothering me. The stretching seemed to have helped with that a lot.
L's party was at Gymboree and I have never seen so much chaos in my life. The 2 teachers (we had to pay extra and hire a second one bc we had so many kids) were useless. They could not run the party and it ended up being mostly free play. For some reason, walking barefoot on the mats made my bad knee crazy, and then once we got home, it starting having shooting pains.
We just unwrapped all the presents, cleaned the living room and put the kiddies to bed!
I am praying that tomorrow is a better day(probably less fun, but hopefully less pain too!)

Why I Still Don't Think My Psoas Needs to Be Released
Many of you have asked me why I was so adamant about not having my psoas released. No, I am not a psychotic bitch who wanted to make my OS absolutely crazy, or an arrogant PT who thinks she knows everything. Firstly, I knew with all my heart that it was not the root of my problem, so releasing it would not have abolished my pain, and I was 100% correct in that, as was found on arthroscopic examination. Another point, for me, is that I have a hypermobile SI joint, which periodically gives me problems. All of my surrounding muscles are extremely tight, hamstrings, TFL, ITB, Piriformis....My feelings are that my muscles tighten to protect my SI and maintain in correct alignment.
My right hip is the one giving me the psoas issues.This is the "revision hip". This hip now has a tremendous amount of ROM, compared to pre-op, and compared to the contralateral side, since a lot of bone was taken down. Don't misunderstand, I love the way it feels now, but, my body has never functioned in this range before, it "thinks" that I now have an unstable hip, which in comparison to pre-op, I do. My ROM values are normal and not indicative of a hypermobile hip, yet I do not have the neuromuscular control yet to feel stable there. Initially, my adductors were tightening and going into spasm to stabilize. Once I strengthened them well (with a few knee blips along the way), they fared quite well.
Then my ITB went haywire, and still does occasionally, but I had been doing a lot of abductor strengthening as well. Psoas is now playing into this and making sure to get its share of the limelight. I am hoping that my new found love of Pilates will help with the neuromuscular control I need in the right hip, to help the entire joint stabilize and function well.
Lets just say, I did not have this realization, and decided that now would be a good time to have my psoas released, since it is acting up, I would most likely end up even more unstable hip and would probably begin to have problems with my ITB or maybe the adductors again. This would lead me down a vicious path of multiple releases. Even if the releases are not surgical, but rather manual (PT type) releases, things don't always work out so well, as I experienced at another course I took. It is imperative to retrain the muscles properly to avoid this and to re-educate the joint. Neuromuscular re-education needs to be a huge part of rehab, not just bridges, straight leg raises and clam shells.
I know I am having issues because I have not been the most compliant patient with my PT, especially after the MCL incident in December. But, I am pretty fed up with hip issues, so maybe I need to stop writing and get my butt into Pilates gear!!!

Never Stop Learning
As usual, I am very "dysfunctional" when it comes to muscular and alignment issues. I am tight in all the wrong places, weak in all the other places, an overall mess. I started off the weekend with pretty bad back pain, my psoas had kicked i again and was unbelievably tight, pulling on my lumbar spine, topped with being in a different car than my own with poor back support (or poor ability of the driver, i.e. me, to figure out how to properly adjust the seat!!) My psoas got so bad that it became tender to the touch and full of trigger points. This usually pulls me into poor spinal alignment, making everything else go to hell.
So.......it turns out that my "partner"at the course used to work with P, she actually taught P how to release a psoas, irony of ironies. Today, she tried a release on me like no other. At the psoas insertion on the lesser trochanter. WHOA! Pain, but I sucked it up and let her do it, and then, less tender, and less clicking. I think this will be the only time I will have this done as it required her to put her hands in places I would rather not have P at!!! But just another trick to add to my "bag of tricks".
I am still on the fence with my knee. I am having less pain, and is always delayed onset pain. It still feels tight often, and the joint surface feels "weird", still "squishy" and "not right". One week to go before the appointment, still time to change my mind!!!!

Click Clack Moo
Lately, when I read this book, it reminds me of my right hip. See, I have started getting into Pilates. I actually really like it alot. The only issue is that some of the moves, especially with my legs straight when I am on my back, cause my right hip to click. It is my psoas. Hahaha!! I know what you are thinking. Don't worry, it is painless clicking, but I do actually feel the psoas snapping over "stuff". No, I am not getting it released. I think that even if it was a "problem" and became painful, pride would prevent me from EVER getting it released (newer readers check out posts from October and November of 2007 to get my feelings on releasing psoas).
I have had it click every now and then. The left one clicks when I sit on a low stool or chair, or the toilet sometimes. I was doing a move on Wednesday night, you lie on your back, legs straight and up in the air, separate the legs a little, circle them around, then bring them low and together, and finish with them bent at your chest. Every time I circle them, the psoas clicks. It is more annoying than anything. Maybe something I will just have to get used to.
In the end, the cows give up their typewriters in exchange for electric blankets, the duck acts as the neutral party, exchanging notes between the farmer and the cow. The last page of the book is a picture of the ducks in their pond, the page reads "Click Clack Quack".

Just To Keep everyone on the Edge of Their Seats...
Maybe it is wishful thinking (I am rambling again...I know), but before the FAI was bulldozed out, I could literally take my index finger and point to where the pain was. This pain is readily identified with the entire pinky side of my hand, from the tip of my finger to my wrist, right in the joint crease.
Also, pesky adductor is acting up again. I need to get off of the computer and stretch....I did make an appointment to see P on Friday morning, which means I am, once again, not a PT dropout. Lets keep me at PT PRN status. I actually received a prescription from a doctor today which said PT at 'my discretion'! So I am doing the same....but for myself.
I play this game called 'what if I was my own patient', to see how I would handle this muscle issue. Firstly, I would not approve of rock climbing or ice skating yet. Maybe spinning, but low-level. Definitely not what I have been doing some nights, like tonight. Did I mention I hurt more now that I am off of the bike than I did before I got on? Oops, I left out some important details. I also had a crazy day at work plus I cooked dinner for 2 other families tonight. Ok, so maybe I don't practice what I would otherwise preach. But this hip shit is getting really old.
Rough Day in Hipville
Let me backtrack on the craft thing. I am not crafty, at all. I felt so out of place in the craft store, like I needed a few more piercings and Birkenstocks. For the longest time, I have wanted to try a project. It is called BeautiFill. I have an empty bowl with compartments, and you are supposed to fill it. Hard to visualize so I have included pictures. (see below) I decided to fill the bowl with silk flowers, it looks great! The kids were a disaster in the store. They were exhausted and weren't letting me put my things int he cart, crying for stickers, books, candy.....you get the picture. Luckily, we made it out alive, with a few dirty looks from complete strangers, and safely into the car. They both fell asleep in the car!

I left them at home and proceeded to Target (where else), where things began to go achey...again. I even thought about calling P to squeeze me in tomorrow, but being that it is 1AM and I am on the computer, no where near ready for bed, I am glad I did not, I would never get out of bed that early. I am not 100% sure what is going on, and have been pretty busy so haven't been able to sort it out. I think it may be psoas (I hope it is psoas) as opposed to some joint crap. I am no where near ready to deal with any thing possibly going downhill in the joint. Maybe 30 years from now when I begin thinking about my THR or resurfacing, but this better not be an indication that it may be sooner. Obviously, I am sure the FAI has been wiped out. But there is always the fear that the cartilage damage has continued, or that scar tissue is running rampant in there. Or maybe I need to take a step back and say 'Chill Out!' I am only 9 weeks out, there are ups and downs, and this is just a down. It happens. I have had much worse 'downs' in the past. Don't freak out. Clearly the FAI is gone and that is the most important thing. Maybe I do need to haul my a$$ back to PT like a good, compliant patient and stop rock climbing on my off days!
My evening did end on a funny/ embarassing note though! L is into the Cha Cha Slide. The singer goes through a verse where he says 'funky funky funky funky'. We were getting pizza for dinner and L started singing the song, but she leaves out the 'N' in funky, boy was I embarrassed.
L has some new bones she knows, to date she knows: Scapula, humerus, radius, ulna, phalanges, clavicle, sternum, ribs, femur, patella, tibia, fibula, calcaneus, phalanges. Plus Gluteus maximus and hamstrings. Now, when I ask her if she knows what a hip looks like, she does that classic fist with one hand, other hand over it, then she takes the cupped hand and says 'this is the acetabulum'. She is a genius!

The True Test Drive....I give my Hip a B-
I decided to take the kids out for a walk this morning, a real walk, in the double stroller. I don't think we had done that since my surgery, at least not this distance. I bundled everyone up, stuck them inside the stroller sleeping bags and headed out. The stroller must have weighed 100 lbs. They either got really big in the last 7 weeks or the added weight of winter gear is more than I imagined.
I did have some pain during my walk. Nothing crazy, but pain. Mostly the adductor insertion point, or possibly joint pain, it was hard to tell while I was walking and with all the clothes on. Also, my psoas acted up a bit. but now, 6 hours later, I am fine.
Last night, I had some time on my hands so I went through some old posts, August-November. I am shocked, things got pretty bad. How quickly we forget when things are better. How lucky I feel that I don't have that kind of pain and the emotional issues I went through those months, it absolutely sucked! I know that there are so many more people dealing with similar issues who are not so lucky (or persistent, pains in the ass) to get it taken care of so quickly. And even more people still searching for a diagnosis of their hip pain. It makes me sad. At least I hope that with my blog, more people will learn about their pain and symptoms and seek the appropriate care.
I know everyone is wondering what will become of my PT routine. I am a quitter!! I think I am taking a leave of absence for the time being and seeing how I fare. If I need it, I will be taken back in with open arms!! But the next few weeks will be difficult with the nanny issues, travelling etc, and I feel great! So I am taking the liberty of dropping out. I have not done anything on my own. I should, at the least, be working on my adductors since that is my 'weakest link'. I am not. I still have hopes to return to spinning, just as soon as I am not sick anymore. I can't even make it through the day without a nap at this point. I am calling the doctor tomorrow, regardless of anything!!!

Surgery #3...scheduled
I guess they got the message after last night's email!! I received a call this morning from Dr. Kelly at 7:45. I was already exercising, no chance of waking me up at that time! He said based on everything (read older posts if you don't know what 'everything is), he feels confident that surgery will help. He will address the cam and will NOT release my psoas. He wants to release adhesions in the psoas and do a bursectomy, but, and I asked, more than once, this will not require him to release my psoas. He said that he can't in 'good conscious' release it. He even offered to give me the tool he uses to release the psoas to hold, so he has no access to it!!!
No CPM, no Gameready, Brace as needed, not a huge deal! I have to email his PA and remind her about my Percocet/ Zofran cocktail!! I requested the same anesthesiologist who did my first 2, and first case of the day if possible!
I think that covers everything. What am I going to do for 3 weeks? Oh, Spin!!!! I just finished another fabulous ride! It is amazing how much energy I have while on the bike and how I can spin through the pain, I always pay the next day but it is soooo worth it!!!

My Butt hurts
I need to get back into shape though. I have been pushing really really hard on the bike, and I think I am becoming hypoglycemic. I am totally fine while riding, the second I get off, disaster strikes. I am wobbly, dizzy and horribly nauseous. I have been told to eat protein+carbs before riding to avoid this little issue!
Yesterday my right hip was pretty painful following my first ride (it did not prevent me from riding more though!) We will see how today goes. Oh, I did not hear back from Dr. Kelly, shocker!
When P worked on me yesterday, my hip flexors were really really tight, especially on the right! But with some TLC, they loosened up! I am not surprised, cycling is all about those muscles!!
If all goes well, I will ride again tonight!!!

Always a Party at Casa de Susie
To make things more interesting, I had doctor appt #3 today (warning- not hip related). My stomach has been funky the last few weeks, so after countless emails back and forth with my GI doc, he finally told me to come in and see him. After discussing what is going on and trying to piece everything together, he didn't think my last CT scan (in June) made sense based on his exam and my symptoms, so while I waited, he called the radiologist. He re-read the films and changed the diagnosis. So once again, different body part but same issue- everyone reads scans differently. Hopefully, the re-read will work to my benefit, he changed all my meds. It also helped that I was in the office and we were actually having a live conversation as opposed to back and forth over the Internet. So we will see what happens, of course the pharmacy was out of my meds!

The NOT Pre-op Appointment
I want to start off by saying this visit did go well, and I really like Dr. Kelly a lot, I hope I can portray it as such in the dialogue!
First, I saw him in the hall as he was going to see another patient while I was waiting, and he said "Hi Susie", so he def. knows who I am, but then again, I am huge pain in the ass!!!!
He came out after he saw that patient and asked me if I had had the injection, I said yesterday, he wanted to know how it had gone, I told him I really hurt and can't really flex my hip. He thought that might be a good thing because if the psoas was inflamed, it may be reacting to the injection. But he said we will talk more when I get a room.
I only waited 1 hour! He came in, checked the ROM, and strength of a SLR, I could not do it on the right. Then we talked. He said there are 4 possible reasons for a hipscope to fail: 1) residual impingement 2) psoas issues 3) retear of the labral repair 4) severe arthritis. I do not qualify for 3 and 4, so we are left with 1 and 2. He is pretty convinced it is psoas, I don't know how convinced I am. He did say it is definitely doable with a scope, I don't need an open procedure. I asked who does 'opens' here? He said he does with his partner, some other guys in NY, the guys in Switzerland and some in Boston. I told him I sent my stuff to Boston but he told me 'to stick with you'! I had no issue telling him I had gone for a second opinion.
He wants to wait 1 week and see what happens with the injection, he feels that if it is psoas, an injection may take care of it altogether (sound familiar). And, he added, I know you REALLY don't want me to release your psoas!
So I said, you want me to call you next week? Will you actually call me back? He laughed, and said they are trying to work on a better system. Maybe email would be better, but he doesn't always check his email, so I am going to email Ariana his PA, who will pass along the message. Keep your fingers crossed.
I asked if we could just take care of it and schedule surgery, I am sick of living like this, he said that after we see what happens with the injection, he can do it next week. Seriously, I said, what does your operating schedule look like? No, I am serious, I will do it whenever you want. No, come on, seriously, I didn't believe him. He said I have been on enough of a roller coaster ride, he will do it whenever I want!
So, now another week of unknown and then we will see. We did set a tentative date of November 12.

The Joy of Scheduling
She was going to have to call me back to reschedule this, but the problem was that Dr. Kelly was only going to be in the office for half a day the next day. Screw it, I will leave work early, this is always soooooo complicated. It doesn't help that someone else is making appts for me. So while I was away, A called and left me a message with the appt time. I have to call and confirm it, wouldn't it be 'funny' if she gave away the appt bc I didn't call back soon enough!!!

The Plan
He said that the CT scan didn't show much of anything on the femur, the alpha angle was 30, anything under 50 is normal. Usually the CT scan will help determine if there is a bump on the femur, which is why I wanted you to have it, but it is inconclusive (I didn't mention that I was the one who actually wanted the scan, but so be it). The MRI did show the bump, but there are also post-op changes, so it is hard to tell what is there and what isn't. I think that based on your twisting/ popping, it is your psoas.
I said- originally, I had thought that we were going into surgery to address the cam, I asked him to look at the scope pics and see if he could see the bump, I don't think I got a straight answer, but I am not sure. He said he looks for the mechanism of injury and it was not consistent with cam impingement.
I started talking now, I also spoke at intervals when he spoke, but I didn't think I needed to give you a dialogue like last time! I had the popping but then went on crutches and after a few days had all the same pre-op pain.
He said- the psoas could have bled and scarred down to the capsule. He looked through my chart a little, so you had about half a day of relief from the injection?? Hmmmm........I think it may be the cam or the psoas, and as much as none of us wants to, I think we may need to go back in again and address it.
I said- What would you do? He said he would definitely take down the cam and do a bursectomy around the psoas. I said I REALLY don't want you releasing my psoas. He said it may be scarred down to the capsule.
I told him I need him to be sure that it either is or isn't the psoas, bc I REALLY don't want him releasing it. He said the only way to be sure is to have it injected, do I mind doing that? No, what's one more injection at this point.
So I then said, Ok, how would you like to proceed at this point...other than disowning me as a patient? He laughed, and said he likes the challenging ones. I didn't get a chance to ask him if he meant the challenging cases or the challenging patients!!!
So, the plan is to get the psoas injected and then see him within a week to determine if that is the issue.
So, it looks like he is definitely recommending a revision, but is not quite sure what needs to be done.

I Stand Corrected
I woke up this morning at 5:00 to excruciating pain, probably worse than surgical pain, it hurt to move, breathe or think. Walking was the best therapy. I somehow made it out the door and to work, where I explained to P the day's situation. "I am super tight anteriorly, it feels like there is a bungee cord inside my hip that pulls me backwards when I take too big a step on the right, I woke up in 10/10 pain...please help. Oh, can you do a grade 5 mob in a FABER position."
So P checked it out and began doing some anterior mobilizations, it really hurt with each thrust forward, but it felt like productive pain, if that makes any sense at all. And slowly, the capsule loosened up. He did one grade 5 mobilization, but not in FABER, in prone (on my stomach), it was extremely painful, and I tightened up and began guarding, so he had to promise no more grade 5's!
By the time he was done, I felt great. My gait was normal and the stabbing pain was gone. Around 10:30, the tightness came back with a vengeance. But after walking around for a while, it slowly let up. Later on in the day, I felt it tighten up again, but this was so different, it quickly 'grabbed' me and tightened up, but I walked on it and it loosened up again.
When I got home, I went to the pool with L and Jk, and the constant activity was good to keep it loose.
I see a huge problem though for most people and even more so for those in PT. I come in with a different complaint almost every time I have PT, but I can easily pinpoint exactly what is bothering me and we can remedy it. I think this may be a part of why many people are having unsuccessful surgeries, the surgery is successful, but there are so many 'complications' that arise later on, and describing the location and quality of pain is very difficult. I asked P what he thought about that, and he agreed, and said we are 'lucky' bc I am going through it and it will be a huge learning experience for all of us.

Psychotic Psoas
Today is the definitive sign that no amount of pain, limp or discomfort will stand in my way, or maybe it is the definitive sign that I am an idiot. You be the judge! When I was in college, a bunch of us went out to a bar/ pool hall/ arcade etc for the night. My friend and I discovered a fun arcade game called Dance Dance Revolution, the explanation of DDR from wikipedia is as follows: "In Dance Dance Revolution, a player must move his or her feet to a set pattern, stepping in time to the general rhythm or beat of a song. During normal gameplay, arrows scroll upwards from the bottom of the screen and pass over stationary, transparent arrows near the top (referred to as the "guide arrows" or "receptors"). When the scrolling arrows overlap the stationary ones, the player must step on the corresponding arrows on the dance platform."

Since our initial DDR day, my friend and I have grown and matured, and gone on to have 2 children each. today, we took the kids to an amusement park, which happened to have an arcade, which happened to have DDR. We had to play.
We dumped the kids with the husbands and played, and played, and played, until we were both dripping sweat and I was limping! But we had a blast!! The good thing is that other than the psoas trouble, the left hip handled the jumping like a star, no joint pain, no soreness, right was sore though. But we had a blast! I think this is a good sign, P always jokes that 'next time', we will start plyometrics, which would be some jumping and landing...and I laugh at the absurdity. But today, I did it!!!
I am really happy with my left hip right now, I know I am in pain from the psoas, but if I were to judge the success of the surgery now, I would say A+! And DDR helped to prove this!

Psoas Problems
But this muscle pain is awful. I have narrowed it down to the psoas, good old psoas. I feel like it has adhered to something, and I want it to 'pop' and be free. I have been stretching the heck out of it all day and it has helped a little. I also did some soft tissue work on psoas, iliacus and sartorius today, so now it is only psoas that is being a pain.
I would like someone to manipulate this hip anteriorly, but I don't think I will be able to convince P it is a good idea, but I will definitely try on Monday! That way, the joint can be pushed just past the end range and free up my motion a little (hopefully).
I was actually limping earlier today, real, actual limping, how pathetic. I still cannot understand why my muscles decide to turn on me in such ways. I would think getting in the pool would be helpful but it is so friggin' cold, I don't think it would be worth it!! I will continue to do some soft tissue work on it and update on this when I get home Sunday night.

Ewww, Gross
My left psoas has suddenly decided to rear its ugly head, it just tightened up and is limiting my ER. P will definitely have his work cut out for him tomorrow!! Why is that almost everyone with hip labral tears ends up with psoas problems. After my first scope, I had so much tightness for such a long time, my gait went slightly out of whack as I would subconsciously try to have a longer stride to get a tiny stretch in the psoas with each step. It did eventually resolve on its own, and it got bad around 7-8 weeks post-op, same as I am now.
5 more days until my injection...but who's counting!!!!

Dr. Jekyll and Mr. Hyde
I am back and forth, back and forth, and change my mind every hour, and sometimes minute, even seconds. Yes, I am definitely losing it...again (or still???)
I am not sure where I stand, and as Rachel pointed out, this gray area stinks!
I had a long sit down meeting with my 2 PTs, J and P to discuss this as well as the work issues. We agreed to really work the psoas for now, so P really did, all the way to the insertion, which is on the lumbar spine, and I felt good after.
What I had decided this morning was that for now, I will hold off on injecting anything, of course, I had minimal to no pain at the time! I want to try to really work on the psoas and see if it is really the issue. But now, as my pain level is slowly creeping up, I am yearning for an injection!
The nice thing for me is that the pain is intermittent, on the flip-side, when it doesn't hurt, I wonder if I am imagining the painful times, and I decide that I am crazy/ier!!!
I had P take strength and ROM measurements for me. The left side is doing well, almost all back to pre-op levels. Right side has lost strength and ROM since the last time we measured, around 7 weeks ago. I am going to aggressively strengthen over the next few weeks and see if I can build it up again, and try not to give in to an injection too quickly!
