You Know its Not Going To Be A Good Day When......

for starters, you wake up with pain on the inside of your ASIS, on the hip that was operated on 10 months ago. When your back feels so stiff it hurts to lean forward. When you have 3 parties within 4 hours...and you are hosting one of them...and the guest list is comprised of twenty five 4 year olds.

My psoas has been giving me issues lately, on both sides, I think. It is hard to remember sometimes, although today we are talking of the left side. Operated only once in July 2007. Last night I began to diligently exercise adductors and abductors on both sides to help my "ailing" knee and "psychotic psoas". I also have been stretching the hip flexors a lot more lately. But this morning, I felt like I made a 9 month backtrack in muscular issues (Joint is still good, you can all breathe a sigh of relief, joint is actually amazing and I am super thrilled, which is why I save my muscular issue complaints for when they are really bad). My iliacus is a mess for some reason. Iliacus is the top portion of the combined muscle called "iliopsoas", iliacus+psoas=iliopsoas. It is found posterior to the ASIS and can be palpated by bringing your fingers just under the ASIS. I have not attempted to manually work on it, but have been stretching it as much as possible (all hip flexors). I stretched before I took L to ballet, after I picked her up from ballet, while she was at a ballet birthday party, and before we left to go to her birthday party! I still have a lot of pain localized to that area. It is an improvement from this morning, I think I could almost feel it pulling on my vertebrae, and that is why my back was bothering me. The stretching seemed to have helped with that a lot.

L's party was at Gymboree and I have never seen so much chaos in my life. The 2 teachers (we had to pay extra and hire a second one bc we had so many kids) were useless. They could not run the party and it ended up being mostly free play. For some reason, walking barefoot on the mats made my bad knee crazy, and then once we got home, it starting having shooting pains.

We just unwrapped all the presents, cleaned the living room and put the kiddies to bed!
I am praying that tomorrow is a better day(probably less fun, but hopefully less pain too!)

A New Culprit

SARTORIUS!!!!!!!!!!!!!!! I waltzed into PT yesterday 40 minutes late and begged P to take me! He did luckily bc the tightness is making me insane. I aksed him to figure out something new, tear some tissue, break scar tissue, jump on my leg, surgically release it.......I was desperate and annoyed! After he calmed me down, he got to work. He palpated sartorius on each side and found it was more prominent on the right, more tight and more tender. Aha! So off he went to ART it and do his magic. He also continued with the PA mobs in prone with some ER followed by a serious hip flexor stretch with a dose of contract-relax. I felt amamzing when I finally had to get up and see patients. Best I have felt in a while!

Unfortunately, I worked a REALLY long day yesterday to cover someones vacation, and I am working today to cover as well, so after 8 hours straight on my feet, I was in pain and (gasp) limping a little. I am better this morning and getting ready for another long day in the office.

Interestingly, as P was working, he was feeling around my scope sites on my 'tender' area and it has become a little bit numb. Par for the course I guess. It still burns at times as well, and I keep forgetting to try the Lidocaine patches. Lets see if today I can remember!

A Look Into the Past

My neighbor left me a trashy magazine at my door today. We had been discussing the Brittany Spear's sister pregnancy thing and she mentioned that she had bought OK magazine bc it featured a story on it. I read the story (nothing out of the ordinary for a teenage pregnancy) and then read my 2008 horoscope, honestly, this was my horoscope:
Health: Last year exacerbated health issues for many Cancers, mainly due to stress and demanding work schedules. This year, your health improves, and you should have more of that cherished nesting time. Add variety to your workouts.....

I wish I knew this a year ago and I would have been prepared! 2007 brought more doctor's visits, diagnostic tests, radiation exposure and surgeries than many people experience in a year.

I think I am not going to PT on Monday, and will soon take on the official 'dropout' status.I had better keep my mouth closed though. After my second surgery,P and I had discussed 'dropping out' at 6 weeks since I was doing so well but then I had the joint capsule/ psychotic psoas issue, as well as the whole 'revision BS'.

SHIT! I had though that it was 6 weeks out when that happened, and I was free and clear. Seems like it was closer to the 2 month mark......(color draining from my face) that whole episode and the next few days were awful, especially the next morning. But I will keep a positive attitude and keep plugging along.

On a more negative note, I am still soooo sick. I am definitely going to the doctor on Monday, there is no question that I have a raging sinus infection and NEED antibiotics. It is sad that I have an entire arsenal of narcotics here, I could easily maintain a healthy drug habit with all of my pills, but I lack a simple Z-pack!!!

I Still Never Learn

I spent some time tonight reading posts from August, which would put me at a similar point in recovery after my second scope. I should have just copied some of the posts and pasted them here instead of rewriting!!

It seems that my activity level is putting a damper on my pain level. I was pretty sore today, and being at work never helps this. I started to worry but see that the same thing happened in August. I guess what they is true, history does repeat itself!

My day started with pain, I hate waking up in pain, luckily I can't remember the last time this happened. Clearly yesterday's stupidity was not resolved with a good night's rest! I briefly spoke to P about the adductor and thigh pain. We agreed that nothing could be done about the thigh pain (more on it momentarily), but he agreed that the adductors needed strengthening. He decided I should do the Fitter into adduction. I didn't think I could do it, even with low resistance. He said I could and even demonstrated it to me. Ok, didn't look too bad, so I went to try it. First with the left leg. I could barely move the platform across. 'Oh', said P, 'you really are weak'. It was also really painful since all the wrong muscle tried to compensate, I felt like my patella was going to dislocate medially. Not good. So I tried a few on the right. Surprisingly, it was a little better, but still really difficult and hard on the patella. I need a few more weeks and will return to this!

We settled on standing hip adduction with a theraband and sidelying hip adduction with no weights. I was sore the rest of the day.

I was also super drained today, not sure if the pain got to me or what, but I fell asleep at 7:00 and took an amazing power nap while L slept next to me in my bed!

If history repeats itself, the next few weeks will be tough, as I return to life as it was pre-surgery with a newly shaped hip. My muscles will be sore and tight and the joint irritated and painful with my activity. But 5 1/2 months after the second scope, I am doing great and my only complaint is spasmy adductors. I can take some muscle soreness and aching joint!

Back to the pain on my thigh, which has beautifully worked itself onto the bony portion of my butt! Yep, so now it hurts to lie on my back too! Interestingly, the invisible bruise got worse when everything else hurt more, and as I gently leaned on a table or wall, it screamed out to remind me it was there. I will also have to further investigate the pain in the butt and get back to you. I will be going for acupuncture session #3 tomorrow, maybe being relaxed will help the muscles too!!!

Psoas Problems

I am on vacation (yes, again), just for the weekend. So I have practically sat on my ass for 24 hours, so yes, my right hip is in 'labral heaven'. My left hip has decided to be fresh, and it feels like every muscle has gone into spasm. I don't think I did anything too extreme. J was away on business so I did all the packing for this trip, but it didn't bother me (rather, it didn't bother my hip). It started Thursday, my adductors felt tight, and it hurt me to ER the left hip. This is fine, in my book, painful ER can only be from muscular things, and as long as there is no painful IR, I can be free and clear of any joint issues. P worked on the adductors, and left me a nice bruise, but they feel better.
But this muscle pain is awful. I have narrowed it down to the psoas, good old psoas. I feel like it has adhered to something, and I want it to 'pop' and be free. I have been stretching the heck out of it all day and it has helped a little. I also did some soft tissue work on psoas, iliacus and sartorius today, so now it is only psoas that is being a pain.
I would like someone to manipulate this hip anteriorly, but I don't think I will be able to convince P it is a good idea, but I will definitely try on Monday! That way, the joint can be pushed just past the end range and free up my motion a little (hopefully).

I was actually limping earlier today, real, actual limping, how pathetic. I still cannot understand why my muscles decide to turn on me in such ways. I would think getting in the pool would be helpful but it is so friggin' cold, I don't think it would be worth it!! I will continue to do some soft tissue work on it and update on this when I get home Sunday night.

He Called...finally

So after 2 messages an email, and a conversation with his PA (unrelated to this issue) he called me!!
Here's the deal....the MRI report shows a fraying of the labrum, when comparing the MRI images to the scope pics, he says it is normal, and in the area of the debridement. There is no large flap or loose body in the joint. There is no sign of synovitis, but it is usually not reported, and there is some fluid around the psoas. There is also some loss of cartilage in the area of the original tear, which apparently we knew about after the surgery (I don't recall this though).

I can get the psoas injected if I so desire, at the time of our conversation, I was in no pain, now of course I am!! I told him I would prefer to have the joint injected, since I think that is where my pain is, and that I will call him when I go back to work since that is what has been causing me the most pain. I will decide then.
As for now, I am unsure what this all means. Obviously, a frank, obvious retear would be devastating, but I am pretty sure it is not my psoas.
I have PT tomorrow morning, I will have P really check everything out and we can come to a decision (hopefully together!!) about what to do next!

Venting...again

HE DIDN'T CALL AGAIN!
I called and left a message today (thanks for the prompting Jess!!), now I am in desperation mode...I need to know! to make matters worse, I am going out of town tomorrow, and am not sure if I will have cell phone service where I am going, so this will bring me to a week from today for finding out! (cover your ears) AAAAAHHHHHHHHHHHHHHHHHHHHHHHH!

Ok, I feel a little better now that I got that out!!! So, how am I doing? I can't do a straight leg raise...still. I had PT today, and P checked out the SLR issue. I feel pathetic enough about this whole situation, but not being able to do a SLR is just icing on the cake!! Usually, my patients who have trouble with SLR are 90 year old ladies who have had knee replacements!

So P had to help my leg move! And, I could not complete the downward cycle of the SLR because at about 30', I begin to get very unstable in the joint, it feels like the femoral head is jumping inside the joint. P thought maybe my rotators were still too weak to stabilize the joint, so we did some IR/ER exercises and tried the SLR again. One full SLR, up and down, with minimal problems! Whoohoo!! It worked! Great, we both said, then time for SLR #2, my leg wouldn't go!! P tried to help it up, but it just collapsed down to the table, amidst crazy giggling from me. Time to feel pathetic again!

My homework for my vacation is to work on hip flexion in the water, how exciting!!!

My left side was in awful pain last night, which raised my anxiety level just a touch!!! It turned out to be severe muscle tightness, mainly TFL and adductors. I put some lotion on my fingertips and gently glided the length of the tendons for a few minutes, this did the trick! I am unable to inflict upon myself the pain I would inflict upon my own patient with these symptoms, or the pain I knew P would be inflicting upon me this morning, but my pseudo muscle release was enough to allow me to sleep last night!!
